Our office uses an online check in system to streamline completion of required paperwork and minimize contact in the office. You will receive a text message 2 days prior to your appointment to begin the check in process. All forms required for your visit can be completed through the EPION link including the uploading your insurance card.

If you are unable to complete the online check in process, we ask that you help us make your office visit more efficient by printing the HIPPA Signature, Financial Policy and Patient Health History Forms and bringing the completed forms with you to your appointment.
